1. Completed/Signed registration form (1601) - Player's name on the player registration form and copy of proof of age must be the same (DO NOT USE NICKNAMES). – don’t forget signature at the bottom right hand corner - download Form 1601 from www.cysanorth.org.

2. Picture - No hats or shaded glasses, even if they’re prescription - One small current picture - should be a clear front view approximately 1” x 1” inches in size.

3. Proof of Birth - Only if player did not play in our club last year - A birth certificate or birth registration issued by an appropriate government agency, board of health records, passport, California Driver’s License or Identification Card, alien registration card issued by the immigration and naturalization service attesting to age, certification of an American citizen born abroad issued by the appropriate government agency. HOSPITAL, BAPTISMAL OR RELIGIOUS CERTIFICATES WILL NOT BE ACCEPTED.

4. Registration fee - $100.00 ($95.00 for siblings), No Cash

Coaches Checklist

1. Completed/signed registration form (1628) - Form 1628 Team Official Registration and Risk Management Disclosure Form MUST be completed for EVERY coach associated with that team. Download Form 1628 from www.cysanorth.org.

3. Team Seeding Form (bright green)- must be prepared for each team by the coach. Please list additional coaches on back of form. A coach may only be listed once as the coach of record.

7. Scholarship - $100.00 – one per team
